Facilitating smooth transitions to supported independent living for adults is our core focus

Extending those smooth transitions to supported independent living for the most challenging of adults is our niche.

Quite frankly; when everyone else is pulling their hair out with frustration, they call  the Corner House Services.

While the role of an NDIS Support Coordinator is fairly straight forward, you should expect them to be excellent listeners, highly adaptable, genuinely caring and able to provide a variety of options  as solutions to your needs.

What is Support Coordination?
An excellent  Support Coordinator will have these qualities:

  • Follows the NDIS Code of Conduct

  • Is savvy in NDIS language

  • flexible in their approach

  • sincere and compassionate

  • works to build capacity

  • maintains specific goals and objectives of the customer

  • Navigates within the framework of the NDIS  many plans

  • Resilient, creative and persistent as they pursue the best possible outcomes

Support Coordinators Make Sense of Everything!

The NDIS process can be a large undertaking in and of itself.  The Support Coordinator is the person who make every sentence of the process understandable for you.  The Support Coordinator is an invaluable part of the process.   She or he will provide specific answers to specific questions  - from funding to facilities - so that you can achieve the best results in the shortest amount of time.
Having a broad range of in-depth understanding of physical conditions (at hand) as well as an acute understanding of the NDIS, the local community, the school systems, the local city government, the business community and the location of the best ice cream parlor in town are some attributes of a fantastic NDIS Support Coordinator.

This kind of Support Connection follows the same process with emphasis upon:

  • Addressing issues faced with participation and service providers

  • Regular management of the plan to meet your changing needs

  • Management of services from different suppliers

  • Regular monitoring and outcome reporting for the participant/NDIS Specialist Support Coordination

Support connection Intended for coordinated support where a specialist is involved.
 

What is a Local Area Coordinator?

A Local Area Coordinator or a LAC (pronounced L. A. C.) is someone from the NDIS or an NDIS partnered organization who helps you to implement your plan. The NDIS has partnered with three community organizations in South Australia to deliver Local Area Coordination:

  • Feros Care in the Northern Adelaide and Barossa, Light & Lower North Service Areas.

  • BaptCare in the Western Adelaide, Eastern Adelaide, Yorke & Mid North Service Areas.

  • Mission Australia in the Limestone Coast, Murray and Mallee, Adelaide Hills, Fleurieu and Kangaroo Island, Southern Adelaide and, Eyre Western and Far North Service Areas.

Once you have your NDIS plan approved, you will be contacted by your local LAC to have a meeting either over the phone or in person. The LAC will help you understand your plan and help you choose and connect with service providers. The LAC is also your NDIS contact person to discuss any questions about your plan. The support provided by an LAC is generally at a Support Connection level. Please note the LAC does not approve NDIS plans or provide case management support.
 

What is a Support Coordinator?

Support Coordination is an NDIS funded support for people who need extra help implementing their plan. This could be because your plan is more complex and you require a bit more support to get what you need.

 

A Support Coordinator works in a similar way to a Local Area Coordinator and will coordinate services from a range of suppliers or providers, address service delivery issues and develop the capacity and resilience of your support network.

 

A Support Coordinator can also provide case management support if required. Support Coordinators generally work at a coordination of supports or specialist support coordination level.
